Position Summary – The Cumberland Community Health Worker (CHW) serves as a bridge between healthcare providers and the community, promoting health education and access to services. They will engage with individuals and families to assess health needs, provide information as needed. CHWs facilitate communication between patients and healthcare systems, helping to navigate resources and address barriers to care. They will conduct outreach activities, coordinate health screenings, and advocate for community health initiatives.
This is a part-time temporary, grant-funded position with a defined project end-date of October 30, 2026. This is an onsite position that provides services at multiple community locations throughout Cumberland County. The primary work location will vary based on program and operational needs. Regular travel between community sites is required. This position is not based at the organization's home office.
Person Reports To – Vice President, Healthcare/ Healthcare Government Relations Analyst
Essential Functions –
- Conduct community outreach and engagement activities for rural health grant
- Work collaboratively with the project manager and team to maximize productivity
- Collaborate with the whole project team, contributing to the entire project lifecycle
- Document encounters, referrals, and follow-up actions in the case management database
- Maintain confidentiality, comply with HIPAA and agency policies.
- Perform other duties as assigned to support project goals.
- Utilize a tablet, or other company provided technology, to facilitate this program.
Prerequisite Qualifications –
1. Education/Degree Required
- High School diploma required. Working towards an associate or bachelor’s degree in a relevant field preferred.
- Certified Community Health Worker in NJ (preferred).
2. Years of Experience Required
- Entry Level Position
3. Other Requirements
- Valid driver's license and reliable transportation.
- Ability to travel regularly between multiple community locations.
- Ability to work a flexible schedule, including occasional evenings or weekends, if applicable.

Salary Information- In compliance with the NJ Pay Transparency law, the hourly rate for this position is $25.00. NJII considers factors like, but not limited to, job scope, experience, education, skills, internal equity, and market conditions when making offers. This pay rate covers base pay only and excludes bonuses or other incentives.
Final candidates will be subject to a background screening. Employment or continued employment is contingent upon NJII’s acceptance of the results of the background screening.
