About the RolePerformance Home Health (PHH) is seeking experienced and motivated Full-Time Registered Nurses (RN) to join our home health team. The Home Health RN is responsible for providing skilled nursing care to patients in their homes while meeting PHH clinical, documentation, and productivity expectations.This role requires strong clinical skills, accountability, and the ability to work independently while maintaining consistent communication with clinical leadership and the interdisciplinary team.Position Overview:
- Full-time home health RN
- Salary starting at $100,000/year
- Mileage reimbursement
- Weekly pay
- 30 productivity points weekly (on-call rotation)
- Commercial insurance only -No OASIS documentation
- Wound care, wound vac, picc care, port care and more
- Direct patient care, care coordination, and provider communication
Key Responsibilities
- Perform comprehensive patient assessments, including Start of Care (SOC), recertifications, and discharges
- Develop, implement, and update individualized plans of care in coordination with physicians and the interdisciplinary team
- Provide skilled nursing services such as wound care, wound vac management, port care, injections, catheter care, IV therapy, picc line care and patient/caregiver education
- Monitor patient progress and report changes in condition to the physician and clinical leadership
- Ensure accurate, timely documentation in the EMR in accordance with agency policy and regulatory standards
- Obtain, process, and follow physician orders
- Coordinate care with therapists, case managers, and staffing.
- Educate patients and caregivers on disease processes, medications, and self-care techniques
- Adhere to infection control, safety standards, and HIPAA regulations
- Participate in case conferences, in-services, and quality improvement activities
- Meet productivity and visit expectations
Visit Expectations & Productivity
- Meet PHH full-time visit/point expectations on a weekly basis
- Complete visits efficiently while maintaining quality patient care
- Ensure documentation is completed within PHH-required timeframes
- Communicate proactively with leadership regarding scheduling challenges, missed visits, or barriers to care
- Assist with coverage needs as required to ensure continuity of patient care
Required Qualifications
- Active Registered Nurse (RN) license in the state of Texas
- 2 years of nursing experience as RN
- 1 year home health experience as RN
- Strong clinical assessment, critical-thinking, and time-management skills
- Ability to work independently in the field
- Proficiency with EMR systems
- Knowledge of home health documentation and regulatory requirements
- Reliable transportation and valid driver’s license
- Ability to multitask and manage a full caseload
- Knowledge of HIPAA regulations and ability to maintain patient confidentiality
Preferred Qualifications
- Experience with Kantime EMR system
- Wound care and wound vac certified
- IV therapy, infusion, or specialty skill certified
- LVAD knowledge
- Bilingual skills a plus
