Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans.
- Administer medications and injections accurately while adhering to HIPAA and clinical confidentiality policies.
- Manage IV infusions and catheter care with precision, ensuring patient safety and comfort.
- Document medical records thoroughly using EMR (Electronic Medical Records) systems such as Epic, Cerner, or Athenahealth to maintain accurate and up-to-date patient information.
- Assist with complex procedures such as wound suturing, specimen collection/processing, and airway management in accordance with sterile techniques.
- Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health support.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care to ensure seamless patient transitions across settings.
Requirements
- Valid LVN license in good standing with the state licensing board.
- Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for medical documentation and patient data management.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.
- Experience working with diverse populations including geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care.
- Ability to perform IV insertion, phlebotomy, venipuncture, catheterization, and other invasive procedures safely and effectively.
- Excellent patient assessment skills combined with the ability to respond swiftly in urgent or emergency situations such as acute pain management or airway emergencies.
- Knowledge of long-term care regulations and experience working in skilled nursing facilities or assisted living communities is preferred.
