Geriatric Care Manager
Bend Senior Care Management (BSCM) is seeking a compassionate and experienced Geriatric Care Manager to join our team.
We are a locally owned care management practice dedicated to helping older adults and their families navigate the challenges of aging. Our goal is to provide thoughtful guidance, advocacy, care coordination, and support that promotes independence and the highest possible quality of life.
As a Geriatric Care Manager, you will work directly with older adults and their families to assess changing care needs, develop and coordinate care plans, communicate with healthcare providers, connect clients with appropriate community resources, and help navigate transitions between care settings.

Schedule
Office hours are typically Monday through Friday, 8:00 a.m. to 5:00 p.m.
Care Managers also participate in an on-call rotation with other members of the care management team.
Essential Responsibilities
- Conduct comprehensive client assessments to identify individual needs, preferences, risks, and care goals.
- Develop and implement individualized care plans based on assessed needs.
- Coordinate and collaborate with physicians, healthcare providers, senior living communities, community resources, and family members.
- Monitor clients and adjust care plans as health, functional, or psychosocial needs change.
- Maintain timely and accurate client documentation in accordance with HIPAA requirements and professional standards.
- Advocate for clients and families navigating complex healthcare, aging, and long-term care issues.
- Assist clients and families with transitions between home, hospital, rehabilitation, senior living, and other care settings.
- Identify appropriate community services and resources to support client safety, independence, and quality of life.
- Provide meaningful socialization and life-enrichment support when appropriate as part of the client’s overall care plan.
Qualifications
- Bachelor’s degree in Gerontology, Social Work, Nursing, Healthcare, or a related field.
- Minimum of two years of professional experience in healthcare or social services.
- Demonstrated experience working with older adults; five years of experience with the senior population is preferred.
- Strong understanding of the healthcare system and the ability to effectively navigate medical and community-based services.
- Familiarity with senior care settings and services, including assisted living, memory care, skilled nursing, home care, hospice, palliative care, and end-of-life care.
- Excellent communication, organizational, time-management, and problem-solving skills.
- Ability to communicate compassionately and professionally with clients and families during difficult transitions or health crises.
- Ability to work independently, collaborate with an interdisciplinary team, and make sound decisions in complex situations.
- Commitment to client confidentiality and compliance with HIPAA requirements.
- Valid Oregon driver’s license, reliable personal transportation, and ability to travel locally to client appointments and care settings.
- Ability to participate in the staff on-call rotation.
Compensation
$40.00–$45.00 per hour, depending on experience.
