Registered Nurse - Home Health RN Needed.
Professional Home Health Care is hiring an RN to case manage and make home visits in the Denver metro area. This position is a wonderful match for those nurses who value one-on-one client care, independence, critical thinking, and patient teaching.
Experience in homecare preferred but we do offer a great training program if you are new to the field of homecare. New grads welcome!

Qualification:
- Current Colorado Nursing license
- Valid Driver’s license, clean MVR, and proof of auto insurance
- Reliable transportation
- Must be able to pass Criminal Background Check
Responsibilities and Duties include:
- Makes home visits and provides care ordered by the physician.
- Assessed patients’ needs, reports findings to physician, obtains orders for new needs, and implements the care plan.
- Completes required documentation that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements.
- Assures that all documentation is available in the electronic record within the deadlines.
