American Prime Healthcare is seeking a compassionate and dependable Per Diem Licensed Vocational Nurse (LVN) to provide comfort-focused hospice care to patients and families in their homes. The ideal candidate is organized, attentive, and committed to maintaining patients’ comfort and dignity.
Responsibilities
- Provide skilled nursing care under the supervision and direction of a Registered Nurse.
- Perform routine nursing visits in patients’ homes and care facilities.
- Monitor patients’ symptoms, vital signs, medications, and overall condition.
- Report changes in patient condition promptly to the supervising RN or physician.
- Administer medications and treatments as authorized by the plan of care.
- Educate patients, families, and caregivers about medications, safety, and comfort measures.
- Assist with pain and symptom management according to physician orders and hospice protocols.
- Complete accurate and timely clinical documentation.
- Coordinate with RNs, physicians, hospice aides, social workers, chaplains, and other team members.
- Provide compassionate emotional support to patients and families.
- Participate in on-call coverage when scheduled.
Qualifications
- Current, unrestricted California LVN license.
- Minimum of one year of professional nursing experience preferred.
- Hospice, home health, palliative care, skilled nursing, or related experience preferred.
- Current CPR/BLS certification.
- Valid California driver’s license, reliable transportation, and automobile insurance.
- Strong communication, time-management, and documentation skills.
- Ability to work independently while following the established plan of care.
- Familiarity with electronic medical records is preferred.
Schedule
- Per diem/PRN.
- Flexible scheduling based on patient needs and employee availability.
- Occasional evening, weekend, or on-call availability is preferred.
