CNA DUTIES
(duties include, but are not limited to)
Shaving
· Shower days, every Sunday and prn
Baths
· All residents have assigned bath days
· Apply clean clothes and dress resident appropriately
· Change bed linen (sheets, pillow case, comforter) on bath days
· Shave, wash hair and clean behind the ears
· Clean feet especially in between toes
· Check and/or cut nails (Nurses cut nails on residents with Diabetes and on Coumadin)
· Report skin issues to the Nurse
· Use curtain to provide privacy
· Do not leave resident unattended, use shower shoes, seatbelt in shower chair for safety
Shoes
· Use nonskid socks at night while in bed
· If a resident need shoes, notify Nurse, Activities or Social Worker
· Shoes with socks worn at all times unless a Nurse tells you something different
Shower Room
· Clean and disinfect after every shift
· No dirty buckets, linen or trash left in the shower room
· No clothing, shoes or other items stored in bathtubs
· Secure storage cabinets to avoid resident injury
· Report visible signs of mold or film on floor
Rounds
· Every 2 hours and prn
· Even if they are “SELF” you still have to check on them
· Change soiled or stained clothing
· Assist to bathroom or provide incontinent care
· Provide repositioning while in bed or when oob in their wheelchair and broda chairs
· Keep call lights within reach, do not wrap around the siderails, promptly answer calls
· Overall cleanliness of rooms and bathrooms
· Exits are not blocked
· Use of dirty linen carts while making rounds and placing in soiled utility room after use
· Keep clean linen carts covered
· Residents have readable identification bands, notify Nurse or Medical records if to be replaced
· Make rounds with receiving or off going staff
Mealtimes
· Provide hand hygiene for resident prior to eating
· Offer use of shirt protector to avoid staining clothing
· Staff to practice hand hygiene prior to delivering meal tray
· Check meal ticket for diet order and proper consistency
· Ensure resident is upright and awake to avoid aspiration
· Provide tray set up (open milk cartons, use straws unless contraindicated, cut up meat, provide lid for hot liquids such as coffee)
· Offer alternatives or choices if refusing to eat
· Proper documentation of intake
Supplements, Hydration
· 10a, 2p, and 9p
· Should not be left at the nursing station or stocked in the kitchenette
· Report refusals to nurse and chart in the communication binder
· Offer fluids frequently, utilize hydration carts
Water Pitchers
· To be washed and sanitized in the main Kitchen weekly, each facility to establish own schedule
· Change or replace cracked or dirty water pitchers
· Fill water pitchers with ice water at 0600, 1400 and 2200, must have a cover and straw
· Residents who are NPO and are on thickened liquids will not have a water pitcher at bedside
Morning Routines
· Wash face and hands
· Mouth Care
· Clean clothes
· Assist with clean glasses, hearing aids, dentures
· Make the beds or change dirty linen
Devices, Safety precautions, Restraints
· Wheelchairs, broda chairs are to be cleaned with soap and water, scrubbed with brush as needed weekly
· Motorized wheelchairs are to be wiped and cleaned, charged safely at night
· Utilize and apply as ordered – chair/bed alarms, hipsters, soft helmet, low bed, landing pads, contour mattress, seatbelts, air mattress, heel protectors, heel boots, wedges
· Residents with restraints must be monitored frequently and released for positioning/toileting
· 2 staff needed with lift use at all times
Resident Rooms
· Clean bedside and night stands
· Personal hygiene items labeled and bagged
· No items on top of lockers
· No personal care items in chairs
· Closets neat with no excess hangers
· Divider curtains free of rips, tears or stains
· Personal refrigerators are clean, no expired foods and has a temperature log
· No leftover or open foods in room
· Urinals are clean, emptied and labeled
· No items on bathroom windows
· Notify Housekeeping if sharps container in bathrooms are full
Kitchenettes
· Area is clean, no expired foods
· Resident items are labeled
· No employee food items in refrigerator
· Temperature logs are maintained and documented
Proper Attire (Staff)
· Do not wear Airpods or Earbuds while on duty on the floor
· Do not use personal phone in resident care areas
· Wear appropriate royal blue uniform, Identification card must be visible and correctly use masks
· Follow facility protocol regarding shoes, nail and hair
Nursing Station
· Refrain from lounging at the Nurses’ Station, make frequent rounds in the assigned hallways/unit
· All charting has to be done on the Kiosk to avoid crowding the Nurses’ Station
· No eating, no cell phones or personal items at the Nurses’ Station
Get up list
· Residents on the list have to be up; report refusal to the Nurse in charge and chart it in the communication binder
· Know the residents who need to be readied for appointments, therapy treatment, family visits, outings, activities
· Some residents are medically unable to get out of bed, verify with the Nurse
Rosie
· If you don’t know how to use it correctly, seek assistance from the Nurse
· You must Login in on the tablet for data to upload in AHT (If you need a login, notify the Nurse)
· Notify the Nurse of any problems e.g. unable to connect to the internet, cannot log in
Documentation and Charting
· Document promptly and accurately using the Kiosks
· Check for past due on the Assigned care tasks or Scheduled care
· Complete charting prior to completing shift
Stocking
· Team work; Make sure you stock your rooms after each shift
Smoke Breaks
· Follow break times, 2 (15 minute) and 1 (30 minute) during the shift
· Do not go over the allotted time
· Notify Nurse if going on break, sign breaktime sheet
· There must be staff on the unit, residents must not be left unattended
