Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong commitment to providing high-quality patient care in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments. As an LPN, you will play a vital role in supporting patients' health and well-being while collaborating with healthcare professionals to ensure optimal care delivery.
Duties
- Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities.
- Conduct patient assessments and monitor vital signs to evaluate health status.
- Document patient information accurately in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations.
- Collaborate with registered nurses and physicians to develop and implement individualized care plans.
- Assist in triaging patients and managing acute pain as needed.
- Maintain a clean and safe environment for patients by adhering to infection control protocols.
- Participate in discharge planning and case management for continuity of care.
- Educate patients and families on health management strategies and resources available.
Experience
Candidates should possess the following skills and experiences:
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation practices.
- Understanding of anatomy and physiology relevant to patient assessment and care.
- Skills in medication administration, specimen collection/processing, and diagnostic evaluation.
