We are currently hiring Telehealth Care Coordinators to play a pivotal role in implementing telehealth technology, delivering seamless and positive experiences that enhance patient care.
As a Telehealth Care Coordinator, you will serve as the vital link between patients and healthcare providers, ensuring seamless telehealth sessions. You will be driving to different post acute care facilities and private homes in the Tallahassee, FL area . This position is considered part-time to start, but may lead to full-time, for those interested.
Enhance the Senior Telehealth Experience with In-Person Support
Your role involves preparing patients for their virtual post-acute care appointments by providing in-person assistance. This includes addressing concerns, offering technical guidance, and ensuring patients are comfortable and ready to engage. Whether patients are in a hospital, rehabilitation center, nursing home, or their own home, you will deliver hands-on support tailored to their needs.

You'll help seniors navigate the technology, connectivity, and equipment required for successful telehealth appointments, offering clear, compassionate guidance every step of the way.

Our ideal candidate would have
- Patience, empathy, and problem-solving skills when supporting individuals with varying technical abilities and be able to deliver clear, practical solutions with confidence.
- The ability to quickly learn and adapt to new technologies and procedures, showcasing a sharp learning curve and a versatile, flexible mindset.
- Experience in senior care and knowledge of post-acute settings, such as hospitals, rehabilitation centers, or skilled nursing facilities, is highly desirable. A strong understanding of operations and patient care within skilled nursing facilities (SNFs), rehab centers, or similar post-acute environments is strongly preferred.
- Basic understanding of healthcare processes, terminology, HIPAA compliance, and health information systems.
Requirements:
Current CNA or MA Certification with at least 2 years recent experience. PCT and Home Health experience are a plus.
BLS Certification
Knowledge and skills to accurately measure and record vital signs
TB test within the past 6 months
High School Diploma/GED
Clear criminal background check
A valid driver’s license, a reliable vehicle with insurance, and a clean motor vehicle record
