SMHWI an entity of Turnwell Mental Health network helps provide face-to-face and telemedicine family medicine and psychiatry services at clinics and to patients in multiple other organizations, rehabilitation centers, and detox facilities throughout various states.
We are looking for enthusiastic, high-energy, motivated Nurse Practitioner with experience in Telemedicine and Family Medicine to join our team. Interested candidates must have experience, and must be dependable, punctual, and able to communicate professionally.
The FNP will provide general and preventative care, conduct check-ups, treat illnesses, order lab tests, and prescribe medication for children and adults. Providers will be working to assist PHP and IOP patients in a remote setting.
As a Family Nurse Practitioner (FNP), you will provide integrated primary care services to patients enrolled in our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP). Working collaboratively with psychiatrists, therapists, nursing staff, and treatment teams, you will address the medical needs of individuals receiving behavioral health and substance use treatment, ensuring whole-person, coordinated care.
The FNP will provide comprehensive primary care services, including but not limited to:
Performing History & Physical examinations and medical clearances.
Evaluating and treating acute illnesses such as upper respiratory infections, bronchitis, urinary tract infections, minor skin infections, and other episodic medical concerns.
Monitoring and managing stable chronic medical conditions, including hypertension, diabetes, hyperlipidemia, and other ongoing health conditions.
Assessing and managing medication-related side effects for patients receiving psychiatric medications.
Providing in-program medical triage by determining when a patient's medical concern can be safely managed within the program versus when referral to urgent care or the emergency department is clinically appropriate.
Collaborating closely with behavioral health providers to ensure coordinated, patient-centered care.
Patients served within the PHP and IOP programs often transition from residential treatment facilities into a lower level of care and may reside in supportive housing associated with the program. The FNP will also care for individuals who enter PHP or IOP directly from the community without a residential treatment stay.
Appointments will be coordinated by Turnwell in partnership with treatment center teams. Dedicated clinic blocks will be established on consistent days each week to provide continuity of care, with flexibility to accommodate same-day or urgent medical appointments as clinical needs arise.
The FNP will document patient care using Valant and should be comfortable working within multiple electronic health record (EHR) platforms, including Kipu and other commonly used healthcare systems.
This is a full-time position with standard business hours of Monday through Friday, 8:00 AM to 5:00 PM (PST).
During program startup, the anticipated patient volume is approximately 15–20 visits per week. As additional treatment center partnerships are established, patient volume is expected to grow to 50 or more visits per week.

Conduct outpatient evaluations, to include intake assessments & follow-up assessments, diagnosis, treatment, and ongoing support for individuals with varying health conditions.
Create, communicate, and carry out treatment plans with the patient.
Complete full and appropriate documentation to support professional billing for services.
Interpret test results to diagnosis any health issues and provide feedback to patients.
Educate and counsel patients regarding medication compliance.
Prescribe medications when clinically necessary.
Communicate with patients regarding diagnosis and treatment plan.
Communicate with and make referrals to PCPs and/or specialists as appropriate.
Provide services at community partnership locations.
Maintain required licensure, certifications, and CME credits.
Other duties as assigned.
Must possess an active & unrestricted license to practice in the states of CO and CA.
Must possess an unrestricted DEA license in the state(s) of practice.
Preferred to possess a current board certification by a national certifying body as a Board Certified Family Nurse Practitioner.
Eligible for credentialing with all major insurance plans and to participate in Medicare.
Excellent communication, patient relations, time management skills, and safety awareness are required.
Computer literate and experience with Electronic Medical Records.
