Cloud Care Medicine is seeking a dedicated and compassionate Bilingual Primary Care Provider to join our growing healthcare team at our Poinciana, FL location.
This position is open to qualified Nurse Practitioners, Physician Assistants, MDs, and DOs who are committed to delivering high-quality, patient-centered primary care. The provider will care for a diverse patient population, including adult and geriatric patients, with a focus on preventive care, chronic disease management, acute visits, wellness exams, and continuity of care.
The ideal candidate is clinically confident, compassionate, organized, able to communicate effectively in English and Spanish, and able to work independently while collaborating with physicians, clinical staff, specialists, and administrative team members. Key Responsibilities
- Conduct thorough patient assessments, medical histories, and physical examinations.
- Diagnose and manage acute and chronic medical conditions commonly seen in primary care.
- Develop and implement individualized treatment plans, including medication management, lifestyle counseling, and preventive care strategies.
- Provide routine primary care services, including follow-up visits, wellness visits, sick visits, and chronic care management.
- Order, review, and interpret labs, imaging, and diagnostic testing as appropriate.
- Prescribe medications and monitor treatment effectiveness.
- Provide patient education in English and Spanish on preventive health, medication adherence, nutrition, chronic disease management, and lifestyle changes.
- Coordinate care with specialists, home health agencies, facilities, and other healthcare providers as needed.
- Manage referrals, follow-up care, and ongoing continuity of care.
- Utilize the EMR system to document patient encounters accurately, efficiently, and in a timely manner.
- Maintain accurate medical documentation and follow appropriate coding, billing, and compliance standards.
- Ensure compliance with HIPAA regulations and clinic policies.
- Collaborate with physicians, clinical staff, and administrative team members to support quality patient outcomes.
- Participate in a positive, professional, and patient-focused clinical environment.
Requirements
- Active and unrestricted Florida license as one of the following:
- Nurse Practitioner, APRN
- Physician Assistant, PA-C
- Physician, MD or DO
- Ability to provide outpatient primary care services in an in-person clinic setting.
- Bilingual English/Spanish required.
- Strong clinical judgment and ability to manage a diverse patient population.
- Knowledge of primary care, internal medicine, family medicine, preventive care, and chronic disease management.
- Ability to evaluate, diagnose, treat, and manage acute and chronic conditions.
- Proficiency with EMR/EHR systems for documentation and patient management.
- Excellent communication and interpersonal skills.
- Ability to educate patients clearly and compassionately regarding diagnoses, treatment plans, medications, and preventive care.
- Ability to work independently while collaborating with a clinical care team.
- Strong organizational skills and attention to documentation accuracy.
- Commitment to providing compassionate, patient-centered care.
- Spanish-speaking required.
Qualifications
- Board certification or board eligibility in Family Medicine, Internal Medicine, or an appropriate primary care specialty preferred.
- Primary care, internal medicine, family medicine, geriatrics, or outpatient clinic experience preferred.
- Experience managing chronic conditions such as diabetes, hypertension, COPD, CHF, asthma, hyperlipidemia, and other common primary care conditions preferred.
- Experience with Medicare, Medicaid, managed care, and diverse patient populations is a plus.
- Experience with telehealth is a plus.
- Familiarity with CPT coding, ICD-10 coding, quality measures, and appropriate documentation practices preferred.
- Experience coordinating care with specialists, home health, assisted living facilities, skilled nursing facilities, or community resources is a plus.
