Angel Oak Nursing and Rehabilitation Center is seeking a compassionate and detailed oriented Admissions Nurse (RN/LPN) to manage and coordinate resident admissions for our long-term care and rehabilitation facility.
Hours
Monday through Friday 8:30 to 5.
Overview
The Admissions Nurse oversees the clinical admission process, ensuring accurate assessments, proper documentation, regulatory compliance, and a smooth transition for residents and their families.
Duties
- Complete comprehensive admission assessments
- Review and transcribe physicians' orders
- Perform medication reconciliation
- Coordinate with hospital discharge planners and interdisciplinary team
- Ensure compliance with Medicare/Medicaid and CMS guidelines
- Educate and support residents and families during admission
- Maintain accurate clinical documentation in the EMR
Experience
- Valid RN or LPN license in good standing.
- 1-2 years nursing experience preferred (LTC, rehab, or hospital)
- Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth.
- Knowledge of complex medical procedures such as catheterization, suturing, tube feeding, and sterile technique.
- Strong Assessment and documentation skills.
- Knowledge of Medicare/Medicaid regulations preferred.
- Excellent communication and organizational skills.
