Position Summary
We are seeking a licensed Physical Therapist (PT) to join our growing home health team. The ideal candidate brings hands-on experience in home health service delivery and a strong background in residential habilitation or IDD settings. This role involves evaluating and treating patients in their home environments across Delaware County and surrounding areas, coordinating care with an interdisciplinary team, and maintaining full compliance with Pennsylvania Department of Health and waiver program requirements.
Key Responsibilities

Conduct comprehensive physical therapy evaluations in the home setting, including functional assessments, fall risk screenings, and mobility evaluations

Develop individualized plan of care (POC) in collaboration with the patient, family/caregivers, physician, and interdisciplinary team

Provide skilled PT interventions including therapeutic exercise, gait training, transfer training, balance and coordination therapy, pain management, and adaptive equipment recommendations

Document all evaluations, visit notes, progress notes, and discharge summaries accurately and in compliance with Medicare, Medicaid, and applicable waiver program standards

Coordinate with home health aides, nursing staff, occupational therapists, and other clinicians to ensure continuity of care

Educate patients, family members, and direct support professionals (DSPs) on home exercise programs, safety techniques, fall prevention, and use of assistive devices

Participate in QAPI initiatives, care conferences, and team meetings as needed

Support individuals receiving residential habilitation services with goals tied to functional mobility, independence, and community integration

Ensure compliance with Title 28 (Chapters 601/611) regulations governing home health physical therapy services

Complete all required documentation within established timeframes in the agency's electronic health record (EHR) platform
Required Qualifications

Current, unrestricted Pennsylvania Physical Therapist (PT) license in good standing

Graduate of an accredited physical therapy program (Master's or Doctorate)

Minimum 1–2 years of experience in home health physical therapy

Valid Pennsylvania driver's license and reliable personal transportation

Current CPR/BLS certification

Ability to pass Pennsylvania criminal background check, child abuse clearance, and FBI fingerprinting as required under applicable regulations

Strong clinical documentation skills and knowledge of Medicare/Medicaid home health coverage criteria

Excellent interpersonal and communication skills; ability to work independently in a home/community setting
Preferred Qualifications

Experience working in a residential habilitation (res hab) or IDD/intellectual and developmental disability setting

Familiarity with OLTL Community HealthChoices waiver or ODP waiver programs

Experience with Axxess home health EHR platform (strongly preferred)

Knowledge of Pennsylvania Chapter 6100 or 6400 regulatory framework

Experience collaborating with Direct Support Professionals (DSPs) and interdisciplinary hab teams
