As a Nurse - RN, ICM (Intensive Case Management) you will:

Provide case management and behavioral health services for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team.

Assist multidisciplinary team with crisis intervention, psychiatric medical assessments, case management, and medication support services.

Monitor client's response to medications and reports any concerns to medical staff.

Ensure that clients and family members are educated and informed about their medications and illness management.

Monitor health care needs of the clients and obtains assistance from other health care providers as necessary.

Ensure all medications are managed and stored appropriately as defined by local and state regulations.

Intensive Case Management (ICM) is a community-based program serving adults living with serious mental illness who require a higher level of support to achieve stability and independence. The program focuses on individuals with complex needs who may be at risk of hospitalization, homelessness, or disengagement from care.
ICM utilizes a multidisciplinary, team-based approach to provide comprehensive, individualized services in the community. Staff work closely with clients to coordinate care, support access to housing and benefits, assist with medication and treatment engagement, and build skills to promote recovery and independence.
With smaller caseloads and a strong emphasis on outreach, ICM allows staff to develop meaningful relationships with clients while delivering high-quality, person-centered care. This model is designed to improve overall outcomes, reduce crisis utilization, and support long-term community integration.

Licensure: California Registered Nurse (RN) license required and must be kept current during employment
Certification: BLS certification required and must be maintained throughout employment

Additional Qualifications and Skills

Minimum of one year of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred

Able to assist mentally and physically in emergency situations

Ability to thrive in a fast-paced environment is essential

Bilingual Spanish preferred

A valid California driver's license in good standing, proof of insurance, and personal vehicle is required to fulfill the essential functions of this position.

Proficient computer skills, including MS Office

Ability to pass criminal/background clearance
Under the direction of the Nurse Coordinator and Program Management delivers behavioral health nursing care. Ensures health care needs of clients are addressed using the nursing process and according to CRF Policies and Procedures. Assists clients in accessing community medical and psychiatric resources to achieve optimal level of health and functioning in the community.
