We are seeking experienced and Physical Therapists for home visits. PTs are invited to join our exceptional interdisciplinary team of health care providers. Do you have home health experience? Responsibilities
- Completes comprehensive admission assessment including completion of the OASIS, primary and secondary diagnoses supporting the plan of care and performs all initial aspects of care coordination including medication reconciliation
- The admission therapist creates a plan of treatment inclusive of medical orders, nursing interventions and goals, visit frequency and identifies other disciplines that may be needed to meet patient needs beyond the initial referral for service

- Effectively uses the agency’s software programs to plan, deliver and document patient care according to accepted agency and regulatory guidelines
Qualifications
- Minimum of two year experience as a Physical Therapist in Home Health
- A clear, active FL PT License
- Familiar with EMR (WellSky/Kinnser)
- Knowledge of OASIS-E Assessments (Start of Care, Resumption of Care, Recertification's, Discharges, Transfers)
- Knowledge of Medicare conditions of participation
