crown community health center is seeking a qualified master level social workr (msw) to provide social worke services in acommunity metal heath center / partial hospitalization program setting.
The msw will work with adults experiencing depression, anxiety psychosis , bipoler disorder , social isolation , functional limitations family stressors and difficulty managing everyday ADL'S.
The MSW will assist with psychosocial assessment , treatment planning support , therapeutic group services , case coordinator , community resource support, documentation as part of the Responsibilities
*provide social work services to adult cmhc/php clients
*assist with psychosocial assessment and identification of client
*Facilitate social work groups focus on coping skills , communication skills, socialization, emotinal support community recourses, problem solving , relapse prevention ,and independent function.
*provided support Counseling and client education within the scope of practice of MSW.
*Support development and updating of treatment palms collaboration with the multidisciplinary team.
*Document client participation, symtoms, progress, barriers, and response to interventions .
*collaborate with psychiatrist/PMHNP/RN/LCSW occupational therpy, occupational therapsit, activity staff, and administration.
*Participate in treatment team meetings as needed.

Pay: $30.00 - $40.00 per hour
