The Hospice Registered Nurse (RN) is responsible for providing skilled nursing care to hospice patients in their place of residence and coordinating patient care with the Interdisciplinary Team (IDT), physicians, patients, and families. The RN performs comprehensive assessments, develops and manages hospice plans of care, and ensures quality patient care and regulatory compliance throughout the hospice care process.
This role requires strong clinical assessment skills, care coordination experience, and the ability to work independently in a hospice field setting while collaborating with the interdisciplinary care team.

Registered Nurse (RN), licensed per state requirements
Hospice experience preferred
Home Care Home Base (HCHB) experience preferred
Strong clinical assessment, communication, and documentation skills
Experience working within an interdisciplinary healthcare team
Knowledge of hospice regulations, documentation, and care planning preferred
Reliable transportation, valid driver’s license, and current auto liability insurance
Ability to read, write, and communicate effectively in English
Perform initial and ongoing comprehensive nursing assessments for hospice patients
Develop, implement, and update individualized hospice plans of care to ensure quality and continuity of care
Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families
Educate patients and families on disease process, medications, symptom management, and end-of-life care
Determine scope and frequency of nursing services based on patient condition and care needs
Supervise and provide clinical direction to Hospice Aides and LVN/LPN staff
Maintain accurate and timely clinical documentation in the electronic medical record
Communicate changes in patient condition to physicians and interdisciplinary team members
Ensure compliance with Medicare hospice regulations, documentation standards, and agency policies
Participate in case conferences, IDT meetings, and care coordination activities
Monitor assigned cases to ensure compliance with third-party payer and hospice requirements
Promote hospice philosophy and provide compassionate, patient-centered end-of-life care
