A well-established skilled nursing and rehabilitation facility is seeking an organized, detail-oriented Admissions Coordinator to manage the day-to-day admissions process and serve as a key point of contact between the facility, referral partners, and incoming residents and families.
In this role, you will keep the admissions process running smoothly from first inquiry through move-in — coordinating closely with hospital case managers, discharge planners, and internal clinical and administrative teams to ensure every admission is timely, accurate, and well-communicated. This role is ideal for someone who thrives in a fast-paced environment, picks up new systems quickly, and stays on top of multiple moving pieces without losing track of details.
Key Responsibilities
- Serve as the primary point of contact for incoming referrals, managing the full inquiry-to-admission process with timely follow-up and accurate documentation.
- Maintain up-to-date knowledge of bed availability, service offerings, and admission requirements to respond quickly and accurately to referral sources.
- Communicate consistently with hospital discharge planners, case managers, and physicians to coordinate referral details and ensure smooth transitions into the facility.
- Conduct facility tours for prospective residents and families, providing clear information on services, care levels, and the admissions process.
- Partner with clinical and administrative leadership in daily bed management meetings to align on admissions status and facility readiness.
- Coordinate with internal departments to confirm rooms are prepared and care teams are informed ahead of resident arrivals.
- Maintain organized, accurate admissions records and documentation in compliance with facility and regulatory standards.
- Ensure all admissions practices comply with federal, state, and local regulations.
- Escalate and troubleshoot issues that arise during the admissions process, keeping leadership informed of any barriers to timely intake.
Qualifications
- Associate's or Bachelor's degree preferred; relevant experience may be considered in place of a degree.
- Experience in a skilled nursing or long-term care setting strongly preferred; background in admissions, healthcare administration, or a related field a plus.
- Highly organized with strong attention to detail and the ability to manage multiple admissions simultaneously.
- Quick learner, comfortable picking up new software platforms and processes.
- Strong interpersonal and communication skills, with the ability to build trust with referral partners, families, and internal teams.
- Ability to work independently and collaboratively across interdisciplinary teams.
- Proficient in Microsoft Office and comfortable learning healthcare software platforms.
