Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
- Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems and medical documentation protocols
- Provide specialized care for patients with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care and behavioral support
- Support patients with end-of-life care and hospice needs with compassion, dignity, and respect
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
- Maintain patient confidentiality in compliance with HIPAA regulations while communicating effectively with clients and healthcare teams
- Conduct patient observation to identify changes in health status or safety concerns promptly
Skills
- Proven experience working with individuals with developmental disabilities or in assisted living environments
- Strong knowledge of medical terminology, catheter care, and basic life support (BLS) including CPR and first aid certification
- Familiarity with home health agency operations, home & community care practices, and hospice or geriatrics care models
- Ability to operate EMR (Electronic Medical Records) systems accurately for medical documentation and patient records management
- Excellent patient care skills with a focus on personal care assistance, patient transfer methods, and client communication
- Physical stamina for heavy lifting and assisting with mobility tasks using proper techniques such as Hoyer lifts or patient transfer methods
- Understanding of home health regulations to ensure compliance during all caregiving activities
- Experience working with patients who have dementia or Alzheimer’s disease is highly preferred
- Strong organizational skills for managing multiple client needs while maintaining confidentiality under HIPAA guidelines
