JOB: Seeking an experienced Chaplain with a hospice heart. This is a PRN (part time) position working with patients in the Globe/Miami and surrounding areas. The hospice Chaplain role is to meet the patient on their journey and help provide spiritual support.
***Formal Education in social work, seminary, theology or ordained minister is REQUIRED. Experience working with death and dying for individuals/family/caregiver preferred. Compensation is paid per visit + Mileage, and hourly rate bereavement calls.

Responsibilities
- Provide spiritual support and counseling to patients and their families in hospice care settings.
- Collaborate with healthcare teams to develop and implement care plans that respect patient and family rights and preferences.
- Conduct regular home visits and telephone outreach to assess the emotional and spiritual needs of patients.
- Make periodic bereavement calls and share additional resources with families based on the need.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Facilitate memorial services and support groups as needed.
- Ensure a compassionate and respectful environment for all patients and their families.
