Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote comfort and dignity.
- Monitor patient health by observing vital signs, tracking changes, and reporting concerns promptly to healthcare professionals.
- Support medication administration following care plans and HIPAA regulations to ensure safety and compliance.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Operate patient transfer equipment such as Hoyer lifts and utilize proper manual handling techniques for safe patient transfers.
- Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols.
- Document care activities accurately in client records, communicate effectively with team members, and support the development of personalized care plans.
Requirements
- Previous experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Knowledge of dementia, Alzheimer’s care, memory care, hospice care, and neurodevelopmental disorders enhances your ability to provide specialized support.
- Strong skills in patient observation, monitoring vital signs, medication administration, and first aid/CPR certification are essential.
- Experience working with individuals with disabilities or developmental delays, including autism or intellectual disabilities, is advantageous.
- Ability to lift heavy objects safely using proper techniques such as patient transfer methods or Hoyer lifts; manual handling experience is important.
- Excellent communication skills for client interaction and collaboration with healthcare teams while maintaining HIPAA confidentiality.
- Flexibility to work various shifts in home & community care settings; familiarity with social work principles is a plus.
