Responsibilities
- Assess patients' needs and develop tailored treatment plans based on their specific conditions.
- Provide therapeutic interventions to assist patients in achieving their rehabilitation goals.
- Conduct discharge planning to ensure a smooth transition for patients returning home or moving to the next level of care.
- Collaborate with multidisciplinary teams
- Educate patients and their families on techniques to improve daily living activities.
- Timely completion of daily and weekly documentation of patient treatments and progress
Requirements
- Active California OT license
- Excellent communication skills, both verbal and written
- A dedication to ongoing professional development and staying current with best practices
- A devoted love for working with geriatric and post-acute populations
