Position Summary
We are seeking a dedicated, organized, and compassionate Case Manager Team Leader to join our growing behavioral health team. The Case Manager Team Leader is responsible for providing direct case management services while overseeing and supporting a team of case managers to ensure high-quality service delivery, compliance with regulatory requirements, and positive outcomes for clients.
This position requires strong leadership skills, excellent documentation practices, and a commitment to serving individuals and families experiencing mental health, substance use, behavioral, and social challenges.
Essential Duties and Responsibilities: Leadership & Supervision
- Provide day-to-day supervision and support to case management staff.
- Monitor staff productivity, documentation quality, and service delivery.
- Conduct regular case reviews and provide coaching, guidance, and corrective action when necessary.
- Assist with onboarding and training of new staff.
- Facilitate team meetings and promote a positive, collaborative work environment.
- Serve as a resource for staff regarding client care, policies, and procedures.
Case Management Services
- Maintain a caseload of clients as assigned.
- Complete assessments, service plans, and referrals.
- Coordinate care with healthcare providers, schools, community agencies, courts, and other stakeholders.
- Assist clients in accessing medical, behavioral health, housing, employment, educational, and social services.
- Advocate for clients and families to ensure appropriate services are received.
- Conduct home visits, community visits, and office-based services as required.
Compliance & Documentation
- Ensure documentation meets state, federal, Medicaid, and agency standards.
- Review staff documentation for accuracy, completeness, and timeliness.
- Assist with quality assurance reviews and audit preparation.
- Monitor compliance with treatment plans, service authorizations, and agency policies.
- Maintain confidentiality in accordance with HIPAA and applicable regulations.
Program Development
- Assist management in developing and improving case management services.
- Identify barriers to service delivery and recommend solutions.
- Support program growth and community outreach efforts.
- Participate in agency committees and leadership initiatives.
QualificationsRequired
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Behavioral Sciences, or a related field.
- Minimum of two (2) years of case management experience.
- Minimum of one (1) year of supervisory, leadership, or team lead experience preferred.
- Valid driver's license and reliable transportation.
- Ability to pass a criminal background check and drug screening.
Preferred
- Experience working in behavioral health, substance use treatment, child welfare, or community mental health.
- Knowledge of Medicaid documentation and behavioral health regulations.
- Experience with electronic health record (EHR) systems.
- Professional licensure or certification is a plus.

Pay: $35,568.00 - $37,500.00 per year
