Heartland Nursing & Rehabilitation is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team. Evaluate residents with speech, language, cognitive, voice, and swallowing disorders.

Develop individualized treatment plans based on each resident's needs and goals.

Provide skilled speech therapy services to improve communication, cognition, and swallowing function.

Maintain accurate, timely clinical documentation in accordance with regulatory and facility standards.

Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and other healthcare professionals.

Educate residents, families, and caregivers on treatment plans and progress.

Participate in care conferences and interdisciplinary team meetings.

Ensure compliance with all federal, state, and facility regulations.

Master's Degree in Speech-Language Pathology.

Current Illinois Speech-Language Pathologist license.

Certificate of Clinical Competence (CCC-SLP) preferred.

Previous experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.

Strong communication, documentation, and interpersonal skills.
