Empath Trustbridge Hospice is seeking an experienced and compassionate Clinical Admissions Manager (RN) to provide clinical leadership and operational oversight for our Admissions Department. This is a pivotal leadership role for a registered nurse who excels at guiding teams, driving process excellence, and ensuring patients and families experience a seamless, dignified transition into hospice and palliative care. In this pivotal role, you will guide a high-performing admissions team, ensure timely and coordinated referral-to-admission workflows, and serve as a key liaison across clinical, operational, and physician partners. You’ll combine strong nursing judgment with leadership, organization, and a sense of urgency to ensure patients and families experience a seamless, supportive transition into hospice and palliative care.

What you'll Do
As Clinical Admissions Manager, you’ll combine strong clinical judgment with operational leadership to ensure excellence across the admissions experience.
- Provide clinical and operational leadership for the Admissions Department, ensuring high-quality, compliant hospice care
- Oversee referral-to-admission workflows, ensuring timely visits, appropriate staff assignments, and smooth handoffs to care teams
- Monitor and analyze productivity, conversion rates, timeliness, and documentation accuracy to support quality and compliance goals
- Serve as a key liaison for physicians, referral sources, and internal partners, supporting collaborative and responsive admissions processes
- Ensure appropriate staffing, scheduling, onboarding, training, and ongoing support of the Admissions team
- Coach and mentor team members, fostering accountability, compassion, and continuous professional growth
- Partner with senior leadership to address operational opportunities, performance metrics, and continuous improvement initiatives

What You'll Need
- Active Florida Registered Nurse (RN) license required; BSN preferred
- Minimum of one year of leadership experience or demonstrated leadership capability
- Strong communication, organizational, and personnel management skills
- Proven ability to manage multiple priorities in a fast-paced, time-sensitive environment
- Proficiency with clinical and operational systems; Homecare Homebase experience preferred
- Prior experience in hospice, home health, or palliative care is a plus
