Requirements
-
Graduate of an accredited Practical Nursing program

Current New Hampshire LPN License
-
Current AHA BLS Certification
-
Long-term care or skilled nursing experience preferred
Key Responsibilities
-
Provide skilled nursing care in accordance with physician orders, resident care plans, and established standards of care.

Assist with assessing, planning, implementing, and evaluating resident care needs.

Administer medications, treatments, and nursing interventions as ordered.

Monitor resident conditions and communicate changes appropriately to physicians and nursing leadership.

Supervise and support LNAs/LMNAs during assigned shifts.

Assist with resident admissions, transfers, discharges, and care coordination as needed.

Collaborate with physicians, residents, families, and the interdisciplinary care team.

Complete accurate and timely clinical documentation.

Ensure compliance with state regulations, facility policies, and professional nursing standards.
