- Conduct thorough patient assessments, including medical terminology review, physiological evaluation, and behavioral health screening to determine individual needs.
- Develop personalized care plans that incorporate inpatient, outpatient, and post-acute care strategies, ensuring alignment with patient goals and clinical best practices.
- Coordinate discharge planning by collaborating with hospital teams, social workers, addiction counselors, and community agencies to facilitate smooth transitions from hospital to home or other care settings.
- Manage utilization review processes such as intake evaluations, diagnostic assessments, and utilization management to optimize patient outcomes and resource allocation.
- Maintain accurate medical records while adhering strictly to HIPAA regulations to protect patient confidentiality and privacy at all times.
- Provide patient service through active engagement in behavioral health support, mental health counseling, and addiction counseling tailored to diverse populations including pediatrics and adults.
- Support interdisciplinary communication by documenting case progress, updating medical records, and participating in team meetings focused on inpatient, outpatient, or clinic-based care delivery. Experience
- Proven experience working within hospital environments such as Level I Trauma Centers or Level II Trauma Centers is highly preferred.
- Demonstrated background in acute care settings including ICU (Intensive Care Unit), PICU (Pediatric Intensive Care Unit), emergency medicine departments, or surgical units like OR (Operating Room).
- Familiarity with hospital medicine practices, inpatient management, post-acute care coordination, and outpatient clinic operations is essential.
- Strong knowledge of medical terminology, physiology, diagnostic evaluation techniques, and patient assessment procedures is required.
- Prior experience in behavioral health or mental health counseling combined with social work expertise will enhance your effectiveness in this role.
