Position Summary
The Hospice RN Case Manager is responsible for coordinating and managing the care of hospice patients and their families in accordance with the interdisciplinary plan of care. The RN Case Manager provides skilled nursing assessment, symptom management, patient and caregiver education, coordination of services, and emotional support while ensuring compliance with hospice regulations and agency policies.
Essential Duties and Responsibilities
- Completes initial and ongoing comprehensive nursing assessments of hospice patients.
- Develops, implements, and updates individualized plans of care in collaboration with the interdisciplinary group (IDG).
- Coordinates all aspects of patient care with physicians, nurse practitioners, hospice aides, social workers, chaplains, and other team members.
- Provides skilled nursing care, symptom management, pain management, and end-of-life education.
- Monitors patient condition and communicates changes to the physician and IDG team timely.
- Ensures physician orders are obtained, implemented, and documented appropriately.
- Participates in IDG meetings and contributes to patient care planning.
- Educates patients, caregivers, and families regarding disease progression, medications, safety, and hospice services.
- Supervises hospice aides in accordance with hospice regulations.
- Maintains accurate, complete, and timely documentation within the EMR.
- Completes documentation within agency-required turnaround times.
- Participates in on-call rotation as assigned.
- Assists with admissions, recertifications, and PRN visits as needed.
- Maintains compliance with Medicare, state, federal, and accreditation standards.
- Reports incidents, concerns, and changes in patient condition according to agency policy.
- Promotes patient dignity, comfort, and quality of life.
Qualifications
- Current unrestricted Registered Nurse (RN) license in the State of Michigan.
- Minimum of one (1) year nursing experience required.
- Hospice or home health experience preferred.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Ability to work independently and manage time effectively.
- Strong communication, organizational, and critical thinking skills.
Physical Requirements
- Ability to travel to patient homes, facilities, and hospitals.
- Ability to lift, turn, and transfer patients as needed.
- Ability to sit, stand, bend, and walk for extended periods.
- Ability to utilize standard office and clinical equipment.

Performance Expectations
- Maintain professional and compassionate patient care.
- Ensure timely completion of all documentation and visit notes.

- Maintain confidentiality in accordance with HIPAA regulations.
Bonus Details
- Sign-On Bonus: Up to $5,000
- Bonus payout schedule determined at time of hire
- Full-time employment required
- Must remain in good standing throughout payout period
