Growing home health care agency is seeking a compassionate and skilled Registered Nurse (RN) to provide per diem high-quality home health nursing care. The RN will deliver skilled nursing services in patients’ homes, perform comprehensive assessments, coordinate care, and educate patients and families to promote optimal health outcomes and independence.
Responsibilities
- Perform initial assessments, Start of Care (SOC), Recertification (RC), Resumption of Care (ROC), Discharge (DC) and routine skilled nursing visits
- Develop, implement, and update individualized plans of care per physician orders
- Provide skilled nursing services
- Monitor patient condition, recognize changes in status, and report findings to physicians and care team
- Educate patients and caregivers on disease processes, medications, safety, and self-care
- Accurately complete clinical documentation in a timely manner in compliance with Medicare and agency standards
- Maintain patient confidentiality and comply with all state, federal, and agency regulations
Qualifications
- Graduate of an accredited nursing program
- Active Registered Nurse (RN) license
- Current CPR/BLS certification
- Home health experience
- Strong assessment, clinical judgment, and communication skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license

Pay: $130.00 - $150.00 per hour
