This position would cover the North and West part of the Valley
At Catalytic Health Partners we are looking for a Nurse Practitioner to help the neediest in our communities engage with the healthcare system to receive the best care possible. We take care of the highest cost, highest risk patients for health plans. You will work with these members in their homes to assess their social needs, coordinate their care across the continuum, educate them on medical needs, and help them meet their wellness goals. Working as a team with a Medical Assistant, Medical Liaison and Behavioral Health Specialist you will work to meet patients’ needs through an integrated approach. You will work closely with their Primary Care Providers, Specialist, Case Managers, and others to ensure their success. All members are managed for a year with the goal to improve their medical outcomes. We are a company dedicated to improving the lives of others and ensuring they received great care.
Responsibilities:
- Engage patients building strong relationships, complete assessments monthly and maintain their medical records,
- Act as primary leader of a team, review diagnostic tests and monitor vital signs results
- Create care plans incorporating integrated medical practices
- Prescribe medications and reconcile medications
- Maintain accurate encounter records
- Educate patients and their family members around their medical needs
- Collaborate with patients’ providers around care planning and next steps
- Administer transitional care procedures and education
- Oversee Gaps in Care of patients
- Proactively work on social determinant needs and met immediate meets
- Work as a team with Medical Assistants, Medical Liaison, Social Worker and Behavioral Health Specialists to ensure patients receive great care

Skills:
- Proven experience as a Nurse Practitioner/Registered Nurse in a clinic, hospital, ER, or other setting
- Ability to engage and connect with a patient and their family
- Expertise in primary care, acute care, and chronic care practices
- Knowledge of health and patient care regulations
- Excellent communication skills
- Strong ethics
- Team spirit with a positive attitude
- Patience and ability to plan
- Valid nurse practitioner license and AZ Medicaid ID
Day to Day:
Nurse Practitioners at Catalytic Health Partners can expect to spend their day seeing 5 patients in their home each day. An NP will manage a population of 100 members meeting with each of them monthly to ensure their progress to goals and life changes. Working with their team of Medical Assistant, Medical Liaison, Social Worker, and Behavioral Health Specialist they will plan as a team the engagement and coordination necessary to change the overall projection of the patient. Along with their team they will work to ensure members have proper transitional care out of facilities and patient’s specialist and PCP are involved members situation.
This is a hybrid position. Seeing members in home and completing Telehealth from the employees home
Monday to Friday 8am to 5pm
- 5 members seen in person on field days
- 14 members seen telemed on telemed days.
- There may be times we do both in a day
- 1 on call weekend every 10 weeks, just like all others
