Job Title: Clinical Care Navigator
We are seeking a highly organized, proactive, and patient-focused individual to join our team. The ideal candidate will be reliable, accountable, outgoing, and adaptable, with a strong ability to support both clinical and administrative functions. This position plays a vital role in the daily operations of our medical services, ensuring seamless coordination between patients, providers, and referral partners.
THIS JOB REQUIRES MONDAY THROUGH FRIDAY IN PERSON PRESENCE 7:30 AM TO 3:30 PM - YOU WILL LIKELY TRAVEL BETWEEN SOUTH HILL AND NORTH SIDE ALTERNATING DAYS OF THE WEEK. Please do not apply if this does not work for you.
Responsibilities include but are not limited to:
- Provide direct support to providers, facilities, and patients to ensure accurate, efficient, and organized care
- Assist with managing provider schedules and coordinating appointments
- Answer and route patient, facility, and referral calls in a professional and friendly manner
- Update and maintain medical records in compliance with privacy regulations
- Communicate important information, including notifying patients of results as appropriate
- Coordinate communication with community provider groups and manage patient schedule changes
- Collaborate closely with clinical and administrative healthcare staff
- Support patient intake and discharge processes
- Assist with completing and coordinating required clinical and administrative documentation
- Serve as a core point of contact between providers, facilities, and patients
Required Skills:
- Strong attention to detail and organizational skills
- Excellent verbal and written communication skills
- Ability to interact effectively with patients, providers, and healthcare professionals
- Working knowledge of medical terminology
Required Credentials:
- Medical Assistant, Certified Nursing Assistant, Med Tech or Home Care Aid
- Experience performing medical assistant and/or administrative duties in a clinical setting
Preferred Experience:
- Transitional Care Management experience (TCM)
- Chronic Care Management experience (CCM)
Pay: $24.00 - $27.00 per hour
