SummitWest Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled care to patients throughout Mesa County.
This position offers flexible scheduling and the opportunity to build a caseload over time. You'll work independently in patient homes while receiving support from an experienced team of clinicians dedicated to delivering high-quality, patient-centered care designed to improve quality of life.
Responsibilities

Provide skilled nursing services in patient homes

Perform wound care, medication support, and patient monitoring

Observe and report changes or concerns promptly

Document visits accurately and timely in the EMR

Collaborate with RNs, therapists, caregivers, families, and providers

Support patient education and individualized care plan goals
Qualifications

Current Colorado Licensed Practical Nurse (LPN) License

Valid driver's license, reliable vehicle, and auto insurance

IV Certification preferred; not required

Strong communication and accurate documentation

Ability to work independently and adapt professionally in home health environments
