Community Medicine Inc. is a (Non-profit) FQHC LAL primary care health center located in Bellflower, California. We are currently in the process of expanding our services due to increasing demands of the area. We are seeking an experienced NP who is also motivated and eager to service the under-served population in the area. Responsibilities include but not limited to:
- Assists Medical Director in developing clinical practice guidelines.
- Participates in provider and medical care team meetings.
- Interviews patients to obtain history, performs physical examinations, orders lab and other tests, prescribes medications and treatments.
- Provides continuity in managed care for patients with pre-existing long-term problems.
- Makes referrals for secondary and tertiary care.
- Provides health maintenance visits, evaluates immunizations, provides anticipatory guidance and referrals and performs contract yearly physical examinations.
- Documents and codes all patient contact accurately and legibly in the medical record.
- Performs as an active member of the medical team.
- Ability to interact effectively and professionally with people from diverse cultural, socioeconomic, education, racial, ethnicity and professional backgrounds.
- Performs other duties as assigned.
- Diagnose and treat acute, episodic or chronic illnesses
- Perform comprehensive physical examination of patient
- Ability to build rapport with bilingual patients (English/Spanish speaking)
- Ability to thrive in a fast-pace environment
- Ability to use and document in EMR software
Education: Certified, active, and non-restricted Nurse Practitioner License
Experience: Family Medicine or Family Practice desired.
