Job Summary
We are seeking a dedicated and compassionate Housing Navigator - Case Management to join our team and make a meaningful difference in the lives of individuals experiencing homelessness and housing instability. In this vital role, you will serve as a guiding resource, connecting clients to essential housing assistance programs, supportive services, and community resources. Your energetic approach will help empower high-risk populations to achieve stable, safe, and affordable housing while fostering trust and collaboration within diverse communities. This position offers an exciting opportunity to work directly with individuals from underserved backgrounds, supporting their journey toward independence and stability.
Responsibilities
- Conduct comprehensive client assessments to identify housing needs, barriers, and goals using motivational interviewing techniques.
- Develop personalized case management plans that align with clients’ unique circumstances, including support for mental health conditions, addiction issues, and other social service needs.
- Assist clients in navigating complex housing assistance programs such as LIHTC (Low-Income Housing Tax Credit), Section 8 vouchers, and other HUD (Housing and Urban Development) regulations.
- Advocate on behalf of clients through negotiation with landlords, property managers, and service providers to secure housing placements that meet their needs.
- Provide crisis intervention and crisis management support during urgent situations related to homelessness or high-risk circumstances.
- Maintain accurate documentation of client interactions, progress notes, and case plans using property management tools like OneSite or similar platforms.
- Educate clients on fair housing laws, landlord-tenant law, and their rights under Housing and Urban Development (HUD) regulations to promote fair treatment and prevent discrimination.
- Collaborate with community partners, social services agencies, mental health providers, addiction counselors, and property management teams to ensure holistic support for clients.
Qualifications
- High school diploma or GED required; Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field preferred.
- At least 1 year of experience in housing navigation, case management, social services, homelessness services, or a related field.
- Experience working with individuals experiencing homelessness, housing instability, behavioral health needs, or other vulnerable populations.
- Knowledge of CalAIM, Medi-Cal, Enhanced Care Management (ECM), and Community Supports preferred. CalAIM specifically includes Housing Transition Navigation and Housing Tenancy and Sustaining Services.
- Familiarity with local housing resources, affordable housing programs, housing applications, rental assistance, shelters, and community resources.
- Ability to assist members with housing searches, applications, documentation, landlord communication, housing placement, and tenancy stabilization.
- Strong communication, advocacy, organization, documentation, and problem-solving skills.
- Ability to provide person-centered and trauma-informed support.
- Ability to work independently and conduct community-based outreach and field visits.
- Ability to maintain accurate and timely case notes and service documentation.
- Knowledge of Contra Costa County and/or surrounding Bay Area housing resources is a plus.
- Valid California driver's license and reliable transportation preferred/required if the position involves regular field travel.
