CASE MANAGER – RTC (Temporary Opportunity)

Schedule: Primarily Monday–Friday business hours with flexibility for appointments, emergencies, hospitalizations, and urgent resident needs.
POSITION OVERVIEW
Southall Heritage Youth Home (SHYH) is seeking a highly organized, compassionate, and professional Case Manager to coordinate services and support continuity of care for youth residing in our Residential Treatment Center (RTC).
This position serves as the primary coordinator of resident services involving CPS/DFPS, courts, schools, medical providers, psychiatric providers, hospitals, and other community stakeholders. The Case Manager plays a critical role in ensuring residents receive timely services, documentation remains compliant and audit-ready, and communication between systems remains organized and effective.
The ideal candidate is detail-oriented, able to manage multiple priorities simultaneously, exercises sound independent judgment, and is passionate about helping youth achieve safety, stability, and successful outcomes.
This position is currently structured as a temporary full-time opportunity. Depending on organizational needs, program growth, staffing changes, and employee performance, the position may be extended or become a permanent role.
RESPONSIBILITIES
Case Coordination & Service Planning
- Coordinate and monitor individualized service plans and continuity-of-care activities.
- Coordinate physical health, mental health, psychiatric, dental, vision, educational, and specialty services.
- Participate in treatment team meetings, Youth and Family Team (YFT) meetings, and service planning meetings.
- Support discharge planning, transition planning, and aftercare coordination.
CPS, Court & Stakeholder Coordination
- Serve as a primary point of contact for CPS/DFPS caseworkers, attorneys, CASA representatives, schools, and service providers.
- Coordinate court-related requirements, reports, and documentation deadlines.
- Facilitate communication between stakeholders to ensure continuity of services and timely case progression.
- Identify and escalate barriers impacting resident services or treatment progress.
Medical, Psychiatric & Hospitalization Coordination
- Schedule and coordinate psychiatric, medical, dental, vision, and specialty appointments.
- Attend psychiatric appointments and obtain recommendations, follow-up requirements, and provider documentation.
- Coordinate communication with hospitals, psychiatric facilities, and external treatment providers.
- Monitor hospitalization status, discharge planning, and re-entry coordination.
Educational Coordination
- Coordinate school enrollment, attendance, educational supports, and communication with school personnel.
- Attend ARD meetings and educational planning meetings when appropriate.
- Track educational needs and assist in resolving barriers impacting educational success.
Documentation & Compliance
- Maintain accurate, timely, and inspection-ready case documentation.
- Complete case notes, appointment summaries, hospitalization documentation, and stakeholder communication logs.
- Ensure DFPS, court, and organizational documentation deadlines are met.
- Maintain organized and audit-ready resident files.
Analysis, Reporting & Administrative Judgment
- Independently prioritize competing case management responsibilities and deadlines.
- Track service completion, appointment attendance, hospitalization activity, and documentation timelines.
- Identify recurring service coordination barriers and workflow issues.
- Prepare reports regarding outstanding appointments, hospitalization activity, and continuity-of-care concerns.
- Recommend process improvements that support resident outcomes and operational effectiveness.
- Exercise sound independent judgment when prioritizing work, escalating concerns, and coordinating services across multiple systems.
QUALIFICATIONS
Required:
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, or a related field preferred.
- In lieu of a degree, a minimum of two (3) years of experience in residential treatment, child welfare, foster care, behavioral health, case management, or a related human services field may be considered.
- Valid Texas Driver's License and acceptable driving record.
- Strong organizational, communication, documentation, and time-management skills.
- Ability to manage multiple priorities and deadlines simultaneously.
- Ability to exercise sound independent judgment and discretion.
Preferred:
- Experience working in a Residential Treatment Center (RTC), General Residential Operation (GRO), foster care, or child welfare setting.
- Knowledge of DFPS, STAR Health, CPS, educational systems, and trauma-informed care.
- Experience coordinating services across multiple agencies and stakeholders.
- Familiarity with DFPS Minimum Standards and residential treatment environments.
