The Assessor performs a variety of clinical and administrative work when it has been determined through an initial screening that the consumer has mental health or addictive disease concerns. The purpose of the Assessment process is to perform a formalized assessment in order to determine the consumer's problems, strengths, needs, abilities and preferences, to develop a social (extent of natural supports and community integration) and medical history, to determine functional level and degree of ability versus disability, and to develop or review collateral assessment information.
