Schedule: Mid Shift – 5x8-Hour (14:30 – 20:30)
Requirements
- Active New York LPN License
- BLS Certification
- Experience in rehabilitation or long-term care preferred
- Familiarity with PointClickCare charting system preferred
Job Overview
The LPN provides direct and indirect skilled nursing care to adult and geriatric patients under RN supervision. Responsibilities include monitoring patient conditions, supporting rehabilitation goals, assisting with care planning, and collaborating with the healthcare team to ensure optimal patient outcomes.
Key Responsibilities
- Collect and report physical and psychosocial data
- Assist in developing and updating individualized care plans
- Recommend revisions based on patient progress and outcomes
- Deliver safe, culturally competent, age-appropriate care
- Maintain up-to-date clinical knowledge through ongoing education
- Support team collaboration and patient-centered rehabilitation care
