Job Overview
We are seeking a motivated and skilled Travel Home Health Physical Therapist to serve the Wasatch Front region. This dynamic role involves delivering high-quality physical therapy services directly in patients' homes. The ideal candidate will possess a strong foundation in acute care, outpatient therapy, and home health environments, with a passion for improving patient mobility and independence through personalized care plans.
Duties
- Provide comprehensive physical therapy assessments and develop tailored treatment plans for diverse patient populations, including geriatrics, pediatrics, and orthopedics.
- Deliver hands-on therapies such as trigger point therapy and physiotherapy techniques to promote recovery and pain relief.
- Conduct discharge planning to ensure seamless transitions from inpatient or outpatient settings to home or community care.
- Collaborate with interdisciplinary teams to coordinate patient care across hospital, outpatient, and home health environments.
- Maintain accurate documentation of patient progress using electronic medical record (EMR) systems in compliance with regulatory standards.
- Educate patients and caregivers on exercises, mobility strategies, and injury prevention to foster long-term health benefits.
- Stay current with physiology knowledge, medical terminology, and anatomy to ensure evidence-based practice in all treatments.
Requirements
- Valid state licensure as a Physical Therapist with current registration in good standing.
- Proven experience in inpatient hospital settings, outpatient clinics, or home health environments.
- Strong knowledge of care plans, discharge procedures, and community-based therapy practices.
- Familiarity with EMR systems for documentation and patient management.
- Experience working with diverse populations including geriatrics and pediatrics; knowledge of orthopedics and geriatrics is preferred.
- Ability to perform detailed assessments involving physiology and anatomy principles.
- Excellent communication skills to effectively engage patients, families, and healthcare teams.
- Demonstrated ability to manage multiple cases efficiently while maintaining high standards of patient care.
