Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility, including safe manual handling practices like Hoyer lift use.
- Monitor and record vital signs accurately, maintaining detailed medical documentation within electronic health record (EHR) systems while respecting patient confidentiality and HIPAA regulations.
- Support medication administration under supervision, ensuring proper procedures and aseptic techniques are followed.
- Communicate effectively with patients, families, and interdisciplinary team members to coordinate care plans and address individual needs.
- Provide emotional support and comfort measures tailored to each patient’s condition, including dementia care and Alzheimer's care strategies.
- Assist with patient transfers using appropriate patient transfer methods and equipment such as patient service carts or transfer devices.
- Maintain a safe environment by adhering to infection control protocols, practicing basic life support (BLS), and following safety guidelines for heavy lifting and manual handling.
Experience
- Prior experience working as a CNA in hospice or palliative care settings is highly preferred.
- Demonstrated knowledge of medical terminology, anatomy, physiology, and nursing fundamentals related to geriatrics, pediatrics, or behavioral health.
- Experience utilizing EMR/EHR systems such as Epic or Cerner for accurate medical documentation and patient records management.
- Background in long-term care facilities, nursing homes, assisted living communities, or hospital environments—particularly those involving ICU or trauma center experience—will be advantageous.
- Familiarity with specialized care techniques including catheter care, phlebotomy, infection control practices, and use of assistive devices like Hoyer lifts.
- Strong caregiving skills with excellent communication abilities to work effectively with individuals with disabilities or complex health needs.
- Certification in CPR/BLS is required; additional certifications such as medication administration or DME training are a plus.
