Social Worker, Master’s – Hospice Home Visits, full-time float
Faith Hospice is seeking a full-time Master's prepared Social Worker to join our team provide flexible, responsive social work support across the hospice continuum to address emergent needs, facilitate timely admissions, and deliver hospice education in the community.
As a Hospice Social Worker, you will:
- Work a Monday – Friday schedule, 8a – 5pm with participation in the after hours on-call rotation with the Hospice MSW team
o Emergent visits
o Admission sign-ons
o Informational visits
o Ensuring patients and families receive timely access to hospice services and compassionate guidance during critical decision points
- Operate as a resource across teams, supporting staffing gaps, high-volume referral periods, and urgent patient/family needs
The Hospice Social Worker role requires:
- Master’s degree in social work from an accredited school
- Full licensure from the State of Michigan as an MSW (limited license is accepted with the requirement of continuing to work towards full licensure while employed by Faith Hospice)
- Due to the autonomous nature of this position, it also requires a minimum of one year of social work experience in a health care/medical setting with strong knowledge of community-based resources.
- Prior hospice social work experience is highly valued.
- Candidates must also have proof of Michigan driver's license and vehicle liability insurance as well as a reliable means of transportation to make home visits.
