Job Title: Physical Therapist (PT) – SNF / LTC
Location: Hilliard, FL
Assignment Length: 13 Weeks
Schedule: Monday–Friday, Day Shift (8-hour shifts)
Guaranteed Hours: 35 hours/week
Compensation:
- Local Rate: $46/hour
- Travel Rate: $1,946/week
We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) setting. The role involves providing therapy services to geriatric and older adult populations, including both subacute rehabilitation and long-term care patients.
- Evaluate patients and develop individualized treatment plans
- Deliver physical therapy interventions in SNF and LTC settings
- Assess mobility, strength, balance, and functional abilities
- Collaborate with interdisciplinary healthcare teams
- Educate patients, families, and caregivers on treatment plans and exercises
- Maintain accurate and timely clinical documentation
- Monitor patient progress and adjust treatment plans accordingly
- Active Physical Therapist license in Florida (or ability to obtain prior to start)
- Graduate of an accredited Physical Therapy program
- Experience in Skilled Nursing Facility (SNF), Subacute Rehab, or Long-Term Care
- Experience working with geriatric populations
- Strong communication, documentation, and patient care skills
- Ability to work independently and within a team
- Prior travel or contract experience
- Experience with post-surgical, orthopedic, neurological, or complex rehab cases
- Familiarity with SNF documentation systems and compliance standards
- 13-week contract
- Guaranteed 35 hours per week
- Monday–Friday schedule (no weekends)
