Position Overview
We are seeking a Bilingual Spanish Member Assessment Agent to support members through comprehensive health risk assessments and help connect them with appropriate healthcare, care management, and community resources.
The ideal candidate will have experience in healthcare customer service, call center operations, medical assisting, community health, or a similar member-facing healthcare environment. Strong communication skills and the ability to engage members in both English and Spanish are essential.
Assignment Details
Position:
Member Assessment Agent – Bilingual Spanish
Assignment Duration: ASAP through December 31, 2026
Schedule: Monday–Friday, 8:30 AM–5:00 PM, with a 30-minute lunch
First Day Schedule: Monday–Friday, 8:00 AM–5:00 PM
Start Date: Upon successful completion of the background check
Work Arrangement: Primarily remote, with occasional onsite reporting as required
Language Requirement: Fluency in English and Spanish
Primary Purpose
The Member Assessment Agent conducts Health Risk Assessments (HRAs) primarily by telephone to identify members' medical, behavioral, and social needs. The role helps determine which members may benefit from additional care management services and ensures they are connected with appropriate resources and support.
Key Responsibilities
Conduct comprehensive Health Risk Assessments with members primarily by phone.
Collect and document relevant medical and health history.
Identify healthcare needs, barriers to care, and potential risks.
Screen members for social determinants of health.
Educate members regarding available health plan benefits and services.
Identify members who may benefit from care management or additional support.
Refer members to appropriate healthcare, community, and social service resources.
Accurately document assessment findings and member interactions in applicable EMR/CRM systems.
Verify member eligibility and identify applicable special member status or program criteria.
Collaborate closely with Care Management, Member Services, and other internal teams.
Communicate professionally and compassionately with members from diverse backgrounds.
Maintain confidentiality and follow established healthcare, privacy, and documentation requirements.
Ideal Candidate
Candidates with experience in one or more of the following environments are strongly preferred:
Medical Assistant
Certified Nursing Assistant (CNA)
Community Health Worker (CHW)
Healthcare Call Center Representative
Health Plan or Healthcare Customer Service
Population Health
Care Coordination
Member Services
Patient Outreach or Engagement.
