Advanced Practice Nurse (APN) – Per Diem ( NJ)
Status: Per Diem (as-needed, variable schedule)
Location: Telehealth (Jersey)
Position Summary
We are seeking per-diem Advanced Practice Nurses (APNs / FNP, AGNP, PMHNP, or ANP) licensed in New Jersey to deliver high-quality, ADOLESCENT patient-centered care across primary care and behavioral health settings. The APN will perform comprehensive assessments, diagnosis, treatment planning, medication management (within scope), and care coordination—supporting ADOLESCENTS and ADULTS.
Key Responsibilities
Clinical Care
- Perform episodic and follow-up visits (in-person and/or telehealth); obtain histories, conduct focused/complete physicals, order/interpret diagnostics, and formulate differential diagnoses.
- Initiate and manage evidence-based treatment plans; prescribe medications and non-pharmacologic therapies per state scope and collaborative protocols.
- For Adults: Provide behavioral health/SUD screening (e.g., PHQ-9, GAD-7, AUDIT-C, DAST), brief interventions (MI/CBT-informed), relapse-prevention education, and coordinate MOUD/MAT under collaborating physician protocols.
- Deliver preventive care (immunizations, screenings, chronic disease management) and provide patient/caregiver education tailored to literacy and culture.
- Coordination & Documentation
- Coordinate referrals with community resources, specialists, care managers, and recovery/elder-care partners.
- Complete accurate, timely documentation in the EHR; maintain problem lists, medication reconciliation, allergies, and care plans.
- Participate in interdisciplinary huddles, quality reviews, incident reporting, and case conferences as scheduled.
- Quality, Safety & Compliance
- Adhere to HIPAA and, when applicable, 42 CFR Part 2 confidentiality requirements.
- Follow clinical guidelines, standing orders, collaborative practice agreements, and organizational policies.
- Support quality metrics (e.g., HEDIS-like measures, access/throughput, patient experience) and contribute to PDSA/QI initiatives.
Minimum Qualifications
- Education: Master’s or Doctoral degree in Nursing from an accredited program.
- Licensure: Unencumbered APN/NP license in NJ (one or both states acceptable).
- Population Certification: FNP, AGNP, ANP, or PMHNP (ANCC/AANP or equivalent).
- Prescriptive Authority: Active state prescriptive authority; DEA registration (or eligibility) and, if applicable, state CDS registration (e.g., NJ CDS).
- Experience: 3+ years post-graduate NP experience preferred (strong new grads with relevant RN experience considered). Prior experience in primary care, urgent care, behavioral health/SUD, or geriatrics/home care is a plus.
- BLS required; ACLS preferred.
Preferred Competencies
- Strong background working with adolescents
- Competent in integrated care models; comfort with co-managing medical and behavioral conditions.
- Familiarity with MOUD/MAT protocols (e.g., buprenorphine) and harm-reduction principles (training available if needed).
- Proficient with EHRs, telehealth platforms, e-prescribing, e-labs, and e-referrals.
- Strong communication (written/verbal), cultural humility, trauma-informed practice, and teamwork.
- Organized, reliable, and comfortable with fast-paced, per-diem scheduling.
Schedule & Work Expectations (Per Diem)
- Shifts offered based on patient volume and coverage needs; may include day/evening/weekend hours and occasional holidays.
- Typical visit lengths: 20–40 minutes (acuity-dependent); documentation completed same day.
Compliance & Credentialing
- Background check, health clearance (TB/Immunizations), OIG/SAM exclusions check.
- Proof of licensure/certifications, DEA/CDS, NPI, malpractice coverage (organization-provided or attestation, per policy).
- Completion of orientation, HIPAA/42 CFR Part 2, OSHA/bloodborne pathogens, fraud/waste/abuse, and any payer-required modules.
Compensation & Benefits (Per Diem)
- Competitive per-diem rate commensurate with licensure, certifications, and experience.
