Job Summary:
We’re looking for a passionate and personable Nutritionist to join our program. This role is all about taking a holistic approach to Wellness Care—focusing on the whole person, not just a diagnosis.
You’ll be working primarily with Medicare patients (65+), helping them build healthier habits, better understand nutrition, and make realistic lifestyle changes that actually stick. This is a great opportunity if you enjoy building relationships, coaching, and making a real impact in people’s day-to-day lives.
Key Responsibilities:
- Create personalized nutrition plans based on each patient’s needs, goals, and health conditions
- Provide ongoing coaching, encouragement, and accountability through the program
- Help patients understand nutrition in a simple, practical way they can apply daily
- Support patients managing chronic conditions like diabetes, heart disease, and weight concerns
- Track progress and adjust plans as needed
- Work closely with a care team to support overall patient wellness
- Keep clear and accurate documentation
Qualifications:
- Degree in Nutrition, Dietetics, or a related field
- Registered Dietitian (RD) or similar credential preferred
- Preferred: nutrition-related certification or training in chronic disease prevention, health coaching, community health, diabetes education, weight management, or wellness programming.
- Strong communication skills—you’re able to connect, motivate, and educate
- Someone who is compassionate, patient, and genuinely enjoys helping others
