Position - Nurse Practitioner
Department: Clinical Services – Primary Care
Reports To: Medical Director / Clinical Director

Position Summary
The Nurse Practitioner (NP) provides comprehensive primary care services to patients of Rego Park Diagnostic & Treatment Center, an Article 28 Diagnostic and Treatment Center licensed by the New York State Department of Health. The NP evaluates, diagnoses, and manages acute and chronic medical conditions, promotes preventive care, prescribes medications, orders diagnostic testing, and coordinates care in accordance with New York State law and organizational policies.
Essential Duties and Responsibilities:
Clinical Care
· Perform comprehensive patient assessments, histories, and physical examinations.
· Evaluate, diagnose, and treat acute and chronic illnesses within scope of practice.
· Develop individualized treatment plans utilizing evidence-based clinical practice guidelines.
· Order and interpret laboratory studies, diagnostic imaging, and other diagnostic tests.
· Prescribe medications and therapeutic interventions in accordance with New York State law.
· Perform medication reconciliation during patient encounters.
· Provide preventive healthcare services including annual wellness visits, immunizations, screenings, and health counseling.
· Manage chronic diseases and coordinate referrals to specialty providers.
· Provide patient and caregiver education.
· Participate in telehealth services in accordance with organizational policies.
Documentation & Regulatory Compliance
· Complete accurate, timely, and comprehensive documentation in the EMR.
· Maintain documentation that meets Article 28, DOH, CMS, and payer requirements.
· Complete medication reconciliation, preventive care documentation, referrals, and care coordination documentation.
· Maintain complete documentation supporting quality measures and value-based care initiatives, as applicable.
· Maintain patient confidentiality in accordance with HIPAA.
· Participate in clinical audits and documentation improvement initiatives.
Quality Assessment & Performance Improvement (QAPI)
· Participate in the organization's QAPI program.
· Support quality improvement initiatives.
· Participate in chart audits and peer review, as requested.
· Participate in patient satisfaction initiatives.
Patient Safety & Infection Prevention
· Follow infection prevention and control policies.
· Promote a culture of patient safety.
· Comply with OSHA standards and organizational safety policies.
· Participate in emergency preparedness activities.
Collaboration & Professional Conduct
· Work collaboratively with physicians and interdisciplinary team members.
· Communicate effectively with patients, families, and staff.
· Attend required meetings and educational programs, as applicable.
· Maintain professionalism and organizational standards.
Qualifications
· Master's or Doctoral Degree in Nursing from an accredited Nurse Practitioner program.
· Active, unrestricted New York State Nurse Practitioner License.
· Current New York State Registered Nurse License.
· National Board Certification.
· DEA Registration.
· Current BLS Certification.
· Ability to obtain and maintain credentialing and privileging.
Preferred Qualifications
· Experience in an Article 28 Diagnostic & Treatment Center or outpatient practice.
· Experience with geriatric and medically complex patients.
· Experience with eClinicalWorks or comparable EMR.
· Experience providing telehealth services.
Work Environment
· Outpatient Article 28 Diagnostic & Treatment Center.
· Clinical office setting providing scheduled primary care services.
· Frequent use of computers, EMR, and standard medical equipment.
· Regular interaction with patients, families, and interdisciplinary healthcare team members.
· May require prolonged periods of sitting, standing, walking, and performing patient examinations.
Physical Requirements
· Ability to stand, walk, bend, and sit for extended periods.
· Ability to perform patient assessments and office-based procedures.
· Ability to lift up to 25 pounds occasionally.
· Ability to communicate effectively with patients and staff.

Employment Status: Full-Time or Part-Time, W-2 Employee
Schedule: Full-Time or Part-Time
Compensation: Commensurate with experience and organizational compensation structure.
