Belleair Health Care Center, a 120 bed skilled rehab facility is looking for a dynamic individual to join our team as social worker. Candidate must have experience in the health care field and knowledge of Medicare and Medicaid regulations. Person must demonstrate excellent customer service, communication and clinical skills. Education and Certification (Preferred): MSW or BSW or Human Services degree
Skills & Requirements: Must possess the ability to read, write and speak the English language; must have strong communication and presentation skills in both group and individual situations; must be patient, friendly, courteous and respectful to residents in any situation, including difficult ones; must be able to make independent decisions when necessary; must have the ability to follow social services department guidelines as outlined by the facility and other regulations governing social services in a long-term care facility; must be able to move frequently throughout the workday; must be able to lift 20 lbs floor to waist; lift 10 lbs waist to shoulder; lift and carry 20 lbs; and push/pull 20 lbs
Experience: Minimum one year experience in a social services position within a health care setting.
Essential Job Duties
Accurately and timely documents all conversations and other interactions with the residents and families as it relates to the care plan and admission assessments and in accordance with all state laws and regulations.
Identifies risk factors for psychosocial deterioration and intervention responses by analyzing documentation and other data.
Enhances the quality of resident lives by providing interventions which assist residents in reaching goals and addressing their social, emotional and physical needs.
Plays a key role in implementation of advance directives decision processes.
Develops a consistent schedule for family council meetings.
Coordinates a formal grievance or complaint process for residents and families.
Actively participates in the new resident orientation program.
Assists the residents and families with change management aspects of becoming a new resident through individual and group facility programs.
Performs discovery meetings with residents and families to learn more about the resident’s social history, interests and hobbies.
Provides counseling with residents and families to determine feelings, fears and anxieties about death or dying, emotional, physical, spiritual and intellectual needs.
Promotes family involvement and develops a supportive family network.
Acts as liaison for social, health and community agencies by providing individual or group assistance to the residents through crisis and change management.
Recruits, selects and orients qualified team members and volunteers for the Social Services Department.
Provides expectations and monitors performance of Social Services Department staff.
Provides information to families regarding community resources for financial needs and to promote emotional stability.
Builds individual rapport with families to promote communication.
Participates in various routine and non-routine meetings to enhance the effectiveness and efficiency of facility outcomes. Other Job Duties
Assists with the process of discharge planning.
Supports the development of goals and objectives by assisting with social assessments and care plans that identify the medical-related social and emotional needs of the residents.
Promotes family involvement in setting goals and objectives for the residents.
Educates residents and families on the importance of following medical recommendations related to care.
Assists in scheduling appointments and transportation for the residents.
Completes forms timely and accurately in accordance with state, local and facility guidelines.
Assists in coordination of care plan meetings with families.
Assists in resolution of family or resident grievances.
Coordinates room changes as needed
