Essential Duties and Responsibilities:
1. Under the supervision of a licensed Physical Therapist (“PT”) and in accordance with established plans of care, provides physical therapy services to patients in their residences.
2. Provides services only after receiving patient-related information from the supervising PT.
3. Actively initiates timely communication with the supervising PT regarding any of the following:
- Significant changes in the medical condition of patients, including any direction to seek unscheduled medical attention
- Problems with the established plan of care including patient tolerance
- Apparent plateaus in patient progress
- Changes in apparent homebound status of patients
- Achievement by patients of established physical therapy goals
- Admissions to hospitals or other facilities.
- Any other reason that may impact patient safety or quality of service
4. Completes all required documentation in a thorough and timely manner.
5. Provides education and training to other staff on transfers, body mechanics, etc.
6. Maintains patient confidentiality per agency policies and procedures.
7. Attends Home Health Team meetings and provides patient reports as necessary.
8. Performs other duties as assigned by supervisor.
Minimum Qualifications:
Qualifications must include the following: graduate of an accredited Certified Physical Therapist Assistant program with current license to practice in the State of Montana; valid driver’s license; reliable transportation; appropriate auto insurance are all required; and two years’ experience as a PTA is preferred.
