We are seeking a dedicated Hospice RN Case Manager to join our interdisciplinary team and make a meaningful difference for patients and families during life’s most important moments.
Position Summary
The Hospice RN Weekend-On Call is responsible for coordinating and providing skilled nursing care to hospice patients in their homes or care facilities. This role partners closely with physicians, social workers, chaplains, aides, and families to ensure comfort-focused, patient-centered care. This job starts on Fridays at 4:30pm and runs continuously until Monday morning 8:00 AM.
There will be a few scheduled visits for declining patients but you will mostly only go out on a visit if a call is received. This will be salaried at a full time position regardless of hours worked.
Key Responsibilities
- Conduct comprehensive nursing assessments and develop individualized plans of care
- Manage a caseload of hospice patients and coordinate interdisciplinary services
- Provide skilled nursing visits, symptom management, and patient/family education
- Collaborate with physicians regarding patient status, medication adjustments, and care planning
- Accurately document clinical visits and updates in the EMR
- Participate in on-call rotation as scheduled
- Support patients and families with compassion, professionalism, and empathy
Qualifications
- Active Registered Nurse (RN) license in South Carolina
- Minimum 1 year of RN experience (hospice or home health experience preferred)
- Strong clinical assessment and time-management skills
- Excellent communication and interpersonal abilities
- Valid driver’s license and reliable transportation
- Ability to work independently while collaborating with a team
