Responsibilities:
- Direct Support: Provide hands-on assistance with personal care, daily living activities, and goal achievement for individuals served.

Case Management: Develop and maintain Individual Support Plans (ISPs), ensuring timely follow-up and delivery of support services.

Team Leadership: Supervise and mentor staff, evaluate performance, and provide contextual training in therapeutic models.

Behavioral Support: Design and implement behavior management interventions in collaboration with the clinical team.

Program Oversight: Act as the site’s contact person in the absence of management, ensuring smooth operations and program integrity.

Community Engagement: Collaborate with families, guardians, and external agencies to promote individual progress in home, work, and community settings.
Qualifications:

Bachelor’s degree preferred.

1-3 years of related experience, including staff supervision and case management.

Valid standard driver’s license (Class C or D).

Expertise in activity planning, behavior management, and therapeutic techniques.

Strong understanding of person-centered planning and daily living assistance.

Ability to foster independence, choice, and growth in a respectful and culturally appropriate manner.

Familiarity with Seven Hills Foundation policies and procedures and program-specific regulations.
