Position Description
Position Overview
We are seeking a compassionate and experienced Qualified Mental Health Professional (QMHP) to provide intensive, crisis-oriented counseling and case management to youth and their families. This role involves direct clinical services, advocacy, coordination with community partners, and active participation in treatment planning and supervision.
Key Responsibilities
- Provide intensive, crisis-oriented counseling and case management to youth and families
- Foster development of coping, problem-solving, and communication skills aligned with individualized service plans
- Develop and implement service plans in collaboration with referring agencies and stakeholders
- Monitor client progress and update plans as needed
- Prepare quarterly progress reports and discharge summaries
- Educate and advocate for families regarding available community services
- Maintain accurate, up-to-date client records and documentation
- Participate in regular clinical supervision and case reviews
- Attend court proceedings and coordinate with referring agencies
- Accompany clients to necessary appointments (e.g., medical, psychiatric, IEP meetings)
- Collaborate with FAPT teams, school officials, probation officers, and other stakeholders
- Build and document relationships with public and private service providers
- Support administrative and director-level initiatives as needed
- Exercise initiative while maintaining communication with supervisors
- Represent the organization at vendor fairs, community events, and outreach opportunities
· Assume self-empowerment, when applicable, to solve problems and act on opportunities, but maintain good communication with the supervisor to be sure of the appropriateness of the actions taken.
Requirements
- Must pass criminal background check and CPS registry screening
- CPR and First Aid certification required upon hire
- Must meet QMHP qualifications to provide Intensive In-Home Services:
- Physician; or
- Master’s degree in psychology or human services field + 1-year clinical experience; or
- Bachelor’s or master’s in social work + 1-year clinical experience with youth; or
- Registered nurse + 1-year clinical experience with youth; or
- Bachelor's in human services or special education + 1 year clinical experience with youth
- CANS / ANSA certified
- Nurturing Parent certification preferred
- Clinical experience includes direct services, supervised internships, practicums, or fieldwork
- Strong listening and communication skills
- Knowledge of child/adolescent development, family dynamics, behavioral management, and crisis intervention
- Familiarity with community resources and services
- Ability to work effectively with clients, families, and multidisciplinary teams
